60 YR OLD MALE WITH COMPLAINTS OF VOMITINGS , DIARRHOEA

 This is online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent.  CASE :

A 60yr old male came to the casuality with chief complaints of  

  1. fever,numbness and tingling sensation in lower limbs since last night 
  2. 3-4 episodes of vomitings and loose greenish stools since today early morning


HISTORY OF PRESENT ILLNESS :

  • Patient was apparently asymptomatic 1day ago. Then he felt tingling sensation and numbness in his lower limbs.
  • Next day ,early in the morning he had 3 episodes of vomitings and passage of loose ,green coloured stools.

PAST  HISTORY :

NO H/O DM ,HTN ,ASTHMA , EPILEPSY , TUBERCULOSIS ,    
  
PERSONAL HISTORY :
  • Mixed Diet
  • Decreased Appetite
  • Adequate Sleep
  • Decreased Micturition 
  •  Regular Bowel Movements 
  • Consumes Alcohol and beedis regularly 

GENERAL EXAMINATION  

The patient was examined in a well lit room after taking consent.

  • The patient is conscious, coherent and cooperative.
  • He is well oriented to time, place and person.
  • He is moderately built and moderately nourished,
  1. NO PALLOR 
  2. NO ICTERUS 
  3. NO CYANOSIS  
  4. CLUBBING IS PRESENT 
  5. NO GENERALISED LYMPHADENOPATHY  
  6. NO PEDAL EDEMA  
    CLUBBING
